Russian investigators suggested human error may have been to blame in the plane crash that killed the Polish president and 95 others, saying Monday there were no technical problems with the Soviet-made plane.

在上面的报道中，human error指“人为失误”，即人的行为失误，就是工作人员在生产、工作中产生的错误或误差。除此之外还有technical error（技术错误），instrumental error（仪器误差）等不同原因的失误。表达“失误”可以用error，也可以用fault，lapse等。网球、手球等的发球失误就是用fault来表示。Lapse指的“失误”一般为小错，例如：a lapse of the tongue（口误）。Error可以指较为严重的错误、罪过，例如：A small, nevertheless fatal error（很小却是致命的错误）。

在波兰总统不幸逝世后，联合国fly the flag at half-staff（降半旗）为他致哀。俄罗斯已经决定将4月12日列为national mourning day（全国哀悼日）。在遗体被辨认出来后，俄罗斯官方在斯摩棱斯克机场为其举行了简短的mourning ceremony（哀悼仪式）。
